The 1st step is to complete an assessment which is a critical component of your treatment plan. A thorough assessment includes understanding the client's history of mental health, how it has affected their life, and how it manifests in their thoughts, feelings, and behaviors. A comprehensive assessment helps the treatment team tailor a plan to our resident's unique needs and experiences.
Our treatment team works collaboratively with each resident to develop a plan that addresses their specific goals and needs. The treatment plan may include individual therapy, group therapy, medication management, and personal care services, and other interventions. The team may also collaborate with other healthcare providers and community resources to provide holistic care.
After the treatment plan is completed, it is then implemented and put into action.
The treatment team will monitor the goals (or objectives). It is important that a treatment plan starts with a clear set of goals. Active participation is necessary for the treatment plan to be effective and how each party will work to achieve the resident’s goal(s)and outcomes.
During the evaluation, the treatment team will be evaluating the resident’s progress and outcomes. Documenting progress toward the resident’s goals is one of the most important aspects of a mental health treatment plan.
Deciding what level of care a loved one needs is not always easy, especially when faced with the possibility of having to provide 24-hour home care. It may be difficult to face up to the fact that a loved one needs that level of care and it can be a challenge to find the best care possible. What does the 12 Steps teach you?
The 12 Steps outline a path to spiritual progress through a series of actions designed to elicit what The Big Book of Alcoholics Anonymous refers to as a “psychic change” – a complete mental, emotional, and spiritual shift in perception. We believe the 12 Steps can be a critical element of a long-term recovery program.
What is the goal of a 12-step program?
A 12-step program is a peer-based mutual help program for alcoholism, drug abuse and other addictive and dysfunctional behaviors. The steps are guiding principles that outline how to overcome addiction, avoid triggers, and live a healthy, productive life.
Our 12-step programs, discuss the effects of addiction—physical, mental, and
emotional—and respond with specific actions. Two unique features of a 12-step program are:
A group setting: The 12 steps of recovery are discussed and applied in a recovery group. Above all, members of these groups emphasize self-admission of the problem they are recovering from.
Sponsorship: A sponsor is a more experienced recovering addict who guides new members of the group, much like a mentor. According to the related 12-step program of Narcotics Anonymous, “A sponsor is simply another addict in recovery who is willing to share his or her journey through the Twelve Steps.”
Personal Care Services are provided to Residents requiring (ADL) Activities of Daily Living assistance with all, or some of the following: Bathing, Showering, Grooming, Feeding & Hydration.
Medication training and support services include education and guidance regarding medications and their potential side effects. These serve as a beginning orientation for the client in comprehending the nature of his/her emotional disturbances or mental illnesses and the significance of medications in assuring increased tenure in the community and symptom reduction.
